Position Description
The Clinical Therapist is responsible for providing a range of services such as clinical assessments, treatment planning, providing individual/family therapy and case management. This position will also provide ongoing mental health and clinical services as needed and maintains client documentation in accordance with SAFY policy.

Essential Responsibilities:


Clinical:


* Administers, scores and interprets assessment instruments.

* Completes any identified diagnostic assessments. Develops and implements Service Plan based on assessments; Provides ongoing mental health/clinical services as needed or are required by contract; Maintains client documentation in accordance with SAFY policy, KY Cabinet, DCBS, and COA standards.

* Provide Intensive Family Preservation Services that are short-term, in-home to families in crisis. Will follow intensive, in-home evidence based service model (Homebuilders Model).

Will provide direct services to the identified clients/families for 24 hours/7 days a week coverage. Services will be provided in the family’s home or other natural setting.

* Direct therapeutic services will be available 24 hours per day for an average of 45 days per family.

* Depending on the type of intervention, services will be provided to no more than two to four families at a time.

* Services are provided at the level of frequency, intensity and duration appropriate to prevent placement or achieve reunification. Services provided are tailored to each family’s needs, problems, strengths, life style and culture. Work effectively in teams to coordinate services and programming for families and children.

* Develops/maintains consultative and liaison relationships with catchments area caregivers (e.g., schools, human services agencies, etc.) in order to cultivate work relations, provide consultative services and/or promote the coordination and development of mental health services.

* Establishes mutually respectful rapport and implements interventions necessary to meet needs of client/collaterals; appropriately sets limits and maintains helping role as a Mental Health practitioner.

* May be trained in Medicaid billing for in home clients.

Compliance/Reporting:


* Maintain accurate and professional documentation in content and appearance as required for each assigned case.

* Submit written reports to supervisor and other parties as required timely and accurately regarding progress of each family’s case.

* Prepare comprehensive reports that adequately convey the necessary information to permit interpretation and decision making. Maintaining client files in a current status is an essential requirement of this position.



Other:


* Establish and maintain a positive working relationship with all levels of SAFY staff by participating in joint meetings, and problem identification and resolution.

* Establish and maintain a supportive and positive working relationship with youth, foster parents, referring agencies, and other SAFY stake holders.

* Attend relevant in-house and external seminars, trainings, conferences and workshops to develop and enhance knowledge in the designated area as well as achieve relevant education hours per year to enhance professional and job related skills.

* Attends required meetings within division, region, and central office (team and/or community partners).

* Participate in assigned committees and chairs if so appointed. If appointed, maintains liaisons with state and national organizations.

* Meets regularly with supervisor to discuss issues and performance.

* Submit monthly/quarterly reports to immediate supervisor.

* Sensitive and responsive to the cultural differences of staff, SAFY youth, foster parents, referral sources and the public.

* May be on call 24 hours or on a rotating basis to address crisis with clients.

Other duties as assigned by supervisor.

* May be required to accept client co-pays at time of service
