Description
As part of the Maryhurst Transition Services, provides continuing therapeutic services for clients transitioning into less restrictive settings. Services will be focused on adjusting to new setting through individual and group work, adjunct sessions with client’s treatment team, and creating and administering prevention plans to reduce amount of placement disruptions and/or AWOL. Participates in treatment planning and evaluation within the context of an interdisciplinary team and individual transition goals. The Transitional Youth Counselor (TYC) will support all programs with initial review of initial strengths and needs of each new client to help support and build rapport. The TYC will start to create a plan for resources alongside the treatment team by participating in the first two CFT’s and subsequent CFTs as needed. The TYC will completes all documentation and creatively supports the provision and progression of services.
Requirements
High School Diploma or equivalent required. A Bachelor’s degree from an accredited university accredited by an accrediting body of the Council on Higher Education Accreditation in a Human Service field is preferred. A minimum of 2 years experience in a mental health program is required. Must possess a valid state driver’s license and be eligible to drive in accordance with the Maryhurst motor vehicle policy.
