Instructions for Submitting Forms
Please use the rubrics/forms provided to you by the School of Education or print copies from this web page to use conducting observations of student teachers and to provide specific feedback on standards and indicators.
Complete the forms and conduct a post conference with the student teacher to discuss strengths and areas for growth.
All parties in the post-conference will sign the documents where indicated.
Make a copy for the student teacher and retain the originals to submit to the university supervisor at the last committee conference, finalizing the placement.
The student teacher will then scan copies into electronic form and use as needed in their e-portfolios.
E-portfolios will be continuously available to the cooperating teacher, university supervisor, and student teacher coordinator within the first two week of the first placement.
